NOTICE OF HIPAA PRIVACY PRACTICES

Ridge RTC is legally obligated to safeguard the privacy of health information that may reveal a patient’s identity, often referred to as “protected health information,” or “PHI”. This encompasses information that we have created or received about your past, present, or future health or condition, the provision of health care to you, or the payment for this health care.
